This is such a tragic problem and you're description is perfect. There was one time I thought I'd bucked the trend as I took an organization from constant firefighting to smooth running and the leadership team could see the difference, thanks to angry customers getting hold of them on the phone every time something went wrong. However, a couple of years later, all that pain was forgotten and they started pushing back on the cost of the tools I added to the pipeline. Their phones never rang any more, so even though they witnessed the transformation, the passage of time brought on this same old problem!
