One boundary test I’d add is revocation after indexing. Index a document, confirm a cited answer, revoke access, then repeat both retrieval and citation lookup before the index refresh finishes. A tenant filter on the initial search is useful, but the citation resolver also needs to recheck current authority. I’d record the document version and policy decision in the audit trail without storing the sensitive excerpt itself. How are you testing that gap?

@[Ivan Rossouw] well, good catch. I’m not testing that case today... the current model is tenant-scoped, so document-level revocation is a real gap. A stale vector hit after deletion should fail during SQL re-hydration, but revocation without deletion needs a fresh authority check both before returning the chunk and again when resolving the citation.
I liked the TOCTOU test you described, and I agree the audit trail should bind document/version and policy decision, not the sensitive excerpt.
