Hallucinations don’t happen because AI is “bad.”
They happen because the workflow has no verification layer.
If I treat AI like an oracle, I get surprises.
If I treat AI like a fast assistant with checks, I get reliability.
The 3-Check System That Stops AI Hallucinations in Workflows
AI hallucinations are not just wrong facts.
They show up in many forms:
- confident but incorrect explanations
- made-up function names or APIs
- fake citations or features
- invented numbers
- assumptions presented as truth
And the scary part is: it often sounds correct.
So I don’t fight hallucinations with better prompts alone.
I fight them with a system.
Check 1: Source Check (Where did this come from?)
Before I trust any claim, I force the AI to label it.
I make it separate:
- Known from provided context
- Derived inference
- Unverified assumption
Because most hallucinations hide inside “assumptions pretending to be facts.”
Rule I follow:
If the AI cannot tell me where a claim came from, the claim is not allowed into the workflow.
Check 2: Constraint Check (Does this violate reality?)
Most errors are not “wrong facts.”
They are wrong fit.
So I check alignment against constraints like:
- product requirements
- business rules
- technical limitations
- time, cost, scope
- security/compliance
Hallucinations collapse quickly when they meet constraints.
Rule I follow:
If the output doesn’t explicitly satisfy constraints, it’s not shippable.
Check 3: Proof Check (Can it be tested or verified?)
This is the final filter.
I force the AI to provide verification, not confidence.
Depending on the task, proof can be:
- a test case
- a reproducible example
- a reference link (if browsing is allowed)
- a calculation
- a quick experiment plan
- a cross-check against another method
Rule I follow:
If it can’t be verified, it can’t be treated as final, only as a draft.
The Leadership Insight
