Really liked the point about alarm fatigue here, Muhammad Ali Khan. It’s scary how silencing one “routine” alert can quietly open the door to a big incident. How are teams actually measuring this human risk today?

@[Danny Jay] Good question Danny! A number of metrices can be defined such as Alert Acknowledgement time, Alert Override rates, Alert acceptance rates, mean time to triage/ untriage, training and efficacy, etc. To mitigate the risk, it requires creating an environment where employees feel safe admitting mistakes and reporting anomalies without fear of judgment, ensuring that potential issues are raised and addressed.
